Dear
Parents/Guardians:
Blessings on
the
beginning of
a new school
year 2008 -
2009. This
year we have
a new
infirmary
area that is
located next
to the
school
cafeteria.
We invite
you to visit
it. This
area is only
for
emergencies
to be
treated
until
parents can
come and
take the
child to his
personal
pediatrician.
If a child
need to be
given a
medication,
parent will
be asked to
sign a
permission
form.
Medication
should be
labeled in
original
bottles.
Medication
with the
name of
brothers or
sisters will
not be
given. A
daily
medication
for a long
term
treatment
requires a
doctor's
order. It is
important to
have your
phone
numbers
verified. If
the child
has Asthma,
it is
important
for the
nurse to
have a
written copy
of your
child's
complete
treatment
for Asthma.
During the
school year,
I will be
making head
inspection
for
Pediculosis.
If positive,
you will be
receiving a
letter to
begin
treatment.
Follow up
inspection
will be
given. On
the third
inspection
if it is
still
positive,
your child
will not be
permitted in
school.
Hypoglicemic
children
must bring a
doctor's
letter in
order to
establish a
plan for his
meal and
snacks. Any
other
medical
condition
should be
notified to
nurse and
teacher.
Medication
for
allergies
are not
given.

student
feels sick,
he/she
should be
sure to tell
the
teacher. If
the illness
requires
more
attention
he/she will
be asked to
go to the
office. The
School will
provide
simple first
aid. The
School is
not
authorized
to give any
kind of
medicine
without
parent's
written
authorization
If a student
needs to go
home or to
be taken to
the
hospital,
the School
will notify
the parent
or
guardian.
Current
emergency
information
must be on
file for
each
student. It
is the
responsibility
of the
parent to
inform the
School of
any change
in this
data.
Upon
your child's
return to
school:
Send him/her
back with a
"Note
to School"
(printed
from the web
site) to be
circulated
by him/her
among
his/her
teachers.
